How to Apply

Candidates for SCRQSA certification should complete the Application for Certification. Check the appropriate category of certification, sign the application, submit the $50 certification fee and all accompanying documentation. 


You must include a current copy of your ARRT, NMTCB, ISCD or RCIS certification if applying for those categories. 

 

If you are a recent graduate of a one-year or two-year program, you will need to provide us a copy of your certificate of completion or degree and/or  or a letter from your program director stating that you have completed the program and are eligible to sit for the state board or national registry.

 

Candidates applying for recertification should complete the Application for Recertification. Check the appropriate category of certification, sign the application, submit the $50 certification fee and all accompanying documentation. 

You must include a current copy of your ARRT, NMTCB, ISCD or RCIS certification if applying for those categories. If applying for limited certification, you must submit copies of the completed continuing education as required.

A $10 late fee will be applied to any recertification more than thirty days late or if your certification has lapsed and needs to be reinstated.
